What to Bring to the Hospital

What you should bring to the hospital is very important for your care. The following is a brief list of items you should have immediately available if you need to go to an emergency department or if an  admission occurs:

  1. Identification Card (driver's license, student ID) and emergency contacts (relatives and friends names and phone numbers)

  2. List of all allergies (Food and Medicine)

  3. List of all current medications. If you do not have a list, place all of the medication bottles in a bag and bring them to the ER.

  4. List of all medical conditions (for example, diabetes, hypertension, peripheral vascular disease)

  5. List all surgeries (all means all, not just the most recent including elective plastic surgery)

  6. Have the available name(s) of the primary care physician and the specialists that treat the patient.

  7. Bring all cards that pertain to insurance coverage.

Admission

​

Patients and their companion/s will be directed to the Emergency Room located at Main Building beside of pharmacy department for thorough medical evaluation. Upon the request of ER nurses and admitting physician, they will be instructed to proceed to the Admitting Office to coordinate the process of admission in detail. Admitting hours are 24/7

​

·         Patient Data Sheet

​

Patients and/or their companion/s will be asked to accomplish the Patient Data Sheet containing pertinent patient medical information. Patients must provide complete and truthful information about their health as these will be used as basis for medical assessment and treatment. This is a very important requirements and appreciate your cooperation

​

·         Patient Admission kit and Bracelet

All admitted patients are given an admission kit which contain a drinking cup, a spoon and fork, tissue paper and soap patient id bracelet is also provided with the full name, age and gender.

 

Important reminder: DO NOT REMOVE YOUR BRACELET UNTIL YOU ARE NOT DISCHARGED IN INDIGENOUS PEOPLES’ HOSPITAL.

 

 

·         Philhealth Checking

 

Kindly inform the admitting officer upon admission if you are eligible to use philhealth kindly proceed Philhealth corner for more details.

 

·         Visiting Hours

 

Visiting hours from:

 

Morning: 9am to 11am

 

Afternoon: 3pm to 5pm

 

Evening: 6pm to 7pm

 

SPECIAL NOTICE:

         BECAUSE OF THE PANDEMIC THE ADMINISTRATION IS STRICTLY IMPLEMENTING THE FOLLOWING:

  • One Watcher Per Patient.

  • No Visitors Allowed

  • Strict Compliance of Hospital Safety Protocols.
